چکیده

Introduction Mechanical ventilation (MV) of patients with Acute Respiratory Distress Syndrome (ARDS) should be performed with a lung protective strategy, since this is associated with better clinical outcomes. Lung protective MV contains the lowering of the plateau pressure (PPLAT) and the tidal volume (VT). Physician’s choice for lung protective MV can be hindered by the consequence of decreased CO2 clearance, i.e. respiratory acidosis. Veno-venous extracorporeal CO2-removal (ECCO2-R) is a recent therapy allowing extracorporeal CO2 clearance and normalisation of pH.

Low flow extracorporeal CO2 removal in ARDS patients: a prospective short-term crossover pilot study

BACKGROUND Lung protective mechanical ventilation (MV) is the corner stone of therapy for ARDS. However, its use may be limited by respiratory acidosis. This study explored feasibility of, effectiveness and safety of low flow extracorporeal CO2 removal (ECCO2R). METHODS This was a prospective pilot study, using the Abylcap® (Bellco) ECCO2R, with crossover off-on-off design (2-h blocks) under ...

Quantification of Carbon Dioxide Removal at Low Sweep Gas and Blood Flows.

Advancement in oxygenator membrane technology has further expanded the boundaries in the clinical application of extracorporeal carbon dioxide removal (ECCO2R). Despite the advent of modern poly-4-methyl-1-pentene (PMP) membranes, limited information exists on the performance of these membranes at low sweep gas and blood flows. Moreover, physiological relationships for CO2 removal at these flow...

Low-Flow Extracorporeal Carbon Dioxide Removal Using the Hemolung Respiratory Dialysis System® to Facilitate Lung-Protective Mechanical Ventilation in Acute Respiratory Distress Syndrome.

Extracorporeal carbon dioxide removal (ECCO2R) permits reductions in alveolar ventilation requirements that the lungs would otherwise have to provide. This concept was applied to a case of hypercapnia refractory to high-level invasive mechanical ventilator support. We present a case of an 18-year-old man who developed post-pneumonectomy acute respiratory distress syndrome (ARDS) after resection...

Ecmo in a diabetic patient with cerebellar infarction and ARDS- A case report

Background: Extra corporeal membrane oxygenation (ECMO) has been considered as a crucial modality for the management of acute respiratory distress syndrome (ARDS). Since ECMO is considered to be an invasive modality and may be associated with some complications, there are strict indications for its use. Here we report successful treatment of a case of severe ARDS with Veno-Venous ECMO. Case pre...

Low-flow CO2 removal integrated into a renal-replacement circuit can reduce acidosis and decrease vasopressor requirements

INTRODUCTION Lung-protective ventilation in patients with ARDS and multiorgan failure, including renal failure, is often paralleled with a combined respiratory and metabolic acidosis. We assessed the effectiveness of a hollow-fiber gas exchanger integrated into a conventional renal-replacement circuit on CO₂ removal, acidosis, and hemodynamics. METHODS In ten ventilated critically ill patient...
